A Sports Physical is required at the beginning of the season to provide a baseline for physical fitness to play a sport. This form may be downloaded and printed to serve as a release for your student to participate in WEB activities.

A waiver for insurance purposes is required each year signed by a player and, if the player is a minor, a parent/guardian. Please download, print, sign, and turn in to a Coach or Board Member. Thanks!
